assist life stage transition

Big life changes can feel stressful — especially when routines, support needs, or living situations shift. Assist–Life Stage, Transition is an NDIS support designed to help you manage change smoothly, build independence step-by-step, and feel more confident during new stages of life.

At Empower Independence Care, we provide practical, person-centred support to help you plan, adjust, and settle into new routines — at a pace that feels comfortable for you.
