At Empower Independence Care, we provide practical, person-centred support to help you plan, adjust, and settle into new routines — at a pace that feels comfortable for you.
Who it’s for
This support may be suitable if you are an NDIS participant and you:
Are moving house or changing accommodation (including shared living)
Are leaving school or starting new day programs/services
Are building more independent living skills
Need help adjusting to new routines, responsibilities, or environments
Want to improve planning, confidence, and decision-making
Feel overwhelmed during change and need structured support
How we help
Supports are tailored to your goals and may include:
Building a clear weekly routine (sleep, meals, appointments, activities)
Support with planning and preparing for a move or transition
Developing everyday independent living skills (home routines, safety, organisation)
Coaching to build confidence in new environments and social settings
Support to attend appointments and community-based activities during transition
Working with your family/carers/support coordinator (with your consent) to keep everything aligned

Funding & availability
Assist–Life Stage, Transition is commonly funded through the NDIS under Capacity Building supports, depending on your plan goals and funding categories.
